Referrals
Most clients are referred to VETMED at the request of their regular veterinarian. It is important that complete records, including all pertinent labwork, radiographs and any other written records are sent to our office prior to your visit.
If you are a veterinarian with an emergency referral case, please call our front office at (602) 697-4694.
Referral forms can be found on this website and filled out online which e-mails directly to our front office staff or you can download the appropriate printable form and fax it to our office at (602) 992-3755.

In order to get started, we will need:
- Basic information about the client;
- History and reason for referral;
- Previous treatments and current medications;
- Some information about the referring veterinarian; and
- All pertinent laboratory reports and radiographs.
